• City Inspection (submitted by City Staff)
• OSBI Background check requested at end of process
• OCPD background check (submitted by City staff)
• Photo ID
• Immigration affidavit (if applicable)
• Zoning verification (completed by City staff)

Required documents for annual renewal

• OSBI background check requested at end of process
• OKCPD background check (submitted by City staff)
• Photo ID
• Immigration affidavit (if applicable or not on file)
• Massage affidavit

*Required documents must be submitted to license staff for OKCPD background check, may take 3-5 business days.
